What the bar is

Attainment is reported as the highest rung a provider actually cleared — 99.99%, 99.9%, 99.5%, 99% or 95% — rather than as a pass or a fail against one number. A single threshold says little: at 99.9% almost every free endpoint fails, and at 99% almost everything passes.

Where too few requests were eligible to judge, the answer is not enough data, never a failing grade. A provider we barely sampled and a provider that failed are different findings, and collapsing them would publish the second when the first is true.
